Confidential Mediation Questionnaire
After scheduling, we ask that you complete and return to us our Confidential Mediation Questionnaire (PDF, 140K), when you send us your deposit, to give us some basic information about your matter.

While not mandatory, this is very important, since your responses let us know in advance about any special circumstances, and how you view the mediation process. This will assist us in making your mediation as productive as possible. If issues or questions arise while completing the Questionnaire that you would like to discuss with the mediator in advance, we encourage you to talk with us.
